معاینه و مشاوره جامع تخصصی بیماریهای لثه
بيماري لثه از يك التهاب ساده لثه( ژنژيويت) در ناحيه ي طوق دندانها ، لثه بين دنداني (پاپيلا)، و يا هر دوي اين نواحي شروع ميشود. ميتواند چند دندان يا كل دندانها را درگير كند. قرمزي لثه، خونريزي موقع مسواك يا نخ دندان، يا خونريزي خودبخود از لثه ها جزو علائم شايع ژنژيويت هستند. معاينه ي پريودنتال شامل بررسي كليه ي اين علائم و ساير نشانه هاي بیماریهای لثه و نسوج اطراف دندانها( نسوج پريودنتال)، است. متخصص پريو در جلسه ي معاينه، علت مشکلات لثه، همچنين ميزان پيشرفت ببماري را ارزیابی کرده و عمق شيار لثه اي ( يا پاكت پريودنتال) را بدقت اندازه گيري و ثبت ميكند. راديوگرافي لازم تهيه ميشود و بر اساس يافته ها، تشخيص بيماري را مشخص كرده و درمانهای مناسب را پیشنهاد میدهد. بسته به شدت بیماری، درمانهای غیرجراحی یا جراحی ممکن است توصیه شود. زماني كه ژنژيويت به موقع درمان نشود پيشرفت ميكند و بافتهاي عمقي تر شامل سطح ريشه دندانها، استخوان نگهدارنده اطراف آنها، و ليگامان اتصال دهنده دندان به استخوان فك دچار تخريب و تحليل ميشوند كه به آن بيماري پريودنتال مزمن گفته ميشود. هدف از معاينه ي پريودنتال آگاهي دادن كامل به بيمار در خصوص وضعيت دندانها و لثه هاست.از آنجا كه سلامت دهان از سلامت عمومي جدا نيست تاريخچه ي دقيقي از شرايط سيستميك بيمار، سابقه ي هر گونه بيماري از جمله ديابت، فشار خون، بيماريهاي قلبي-عروقي ،ريوي، بدخيمي ها، و مصرف داروها تهيه ميشود. همچنين عادات بهداشتي بيمار بدقت بررسي و ثبت ميگردد تا در جلسات درماني به اصلاح و يا تغيير آنها در صورت لزوم پرداخته شود. در جلسه ي معاينه پريودنتال جامع،، تمامي دندانها نيز از نظر وجود پوسيدگي، شكستگي تاج يا ريشه و وضعيت عصب دنداني مورد معاينه قرار ميگيرند و توصيه هاي لازم درماني در طرح درمان كلي براي بيمار مشخص ميشود. متعاقبا پيش آگهي بيماري بطور كلي و نيز براي هر دندان تعيين ميشود .

Gingival disease begins as a simple inflammation of the marginal gingiva (gingivitis) in the area around the tooth neck, the interdental soft tissue (papilla), or both. It can affect a few teeth or all of them. Common symptoms of gingivitis include redness, bleeding upon brushing or flossing, etc. In some advanced cases spontaneous gingival bleeding may occur.
A periodontal examination involves assessing all these symptoms as well as other signs of gingival and periodontal diseases. During the examination, periodontist evaluates the etiology, the progression of the disease, and accurately measures and records the depth of the gingival sulcus (periodontal pockets). Necessary radiographs are taken, and based on the findings, the diagnosis is determined, and appropriate treatments are suggested.
Depending on the severity of the disease, non-surgical or surgical treatments may be recommended. If gingivitis is not treated in time, it progresses, leading to the destruction of deeper tissues, including the root surface of the teeth, the surrounding supporting bone, and the ligament connecting the tooth to the jawbone( Periodontal ligament), a condition known as chronic periodontal disease.
The purpose of a periodontal examination is to fully inform the patient about the condition of their teeth and periodontium. Since oral health is closely linked to overall health, a detailed medical history is taken, including any systemic conditions such as diabetes, hypertension, cardiovascular diseases, respiratory disorders, malignancies, and medication usage. The patient’s oral hygiene habits are also carefully assessed and documented to be addressed or modified if necessary during treatment sessions.
In a comprehensive periodontal examination, all teeth are also checked for decay, crown or root fractures, and endodontic problems. Necessary treatment recommendations are incorporated into the overall treatment plan, and a prognosis is determined both generally and for each tooth individually.
